Steps to New Patient Registration

We are using a new online service called Register with a GP surgery that makes it easier to register with this GP practice.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Step 1
Check you are within the practice's catchment area
Each practice has a catchment area within which they can accept new patients.

Step 2

Complete Registration Form
The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery. The New patient questionnaire will ask you for some basic, lifestyle and allergy questions, so we can provide the best service possible from the start.

The form is not electronically sent back to the practice, it is merely completed on screen. Without a correctly completed and signed form the practice can not proceed to register you as a patient.

Once you have completed and signed the form bring the form along to the surgery to be registered with the practice
